Updated US Guideline on Pharmacologic Prevention of Migraine in Adults


The US updated evidence-based recommendations provide clinicians with comprehensive guidance on when to initiate pharmacologic migraine prevention, how to select an appropriate preventive medication, and how to monitor treatment effectiveness, adverse effects, and medication discontinuation in adults. Developed by a multidisciplinary panel using a systematic review of studies published through June 6, 2024, the guideline integrates evidence from the systematic review, related evidence, principles of care, and evidence-based inferences. The recommendations also address medication selection in specific clinical situations and populations, including patients with fibromyalgia, obesity, or hypertension; older adults; pregnant or lactating patients; sex-related considerations in medication choice; and patients with medication overuse, providing an updated framework for individualized pharmacologic migraine prevention.
