Prediabetes Remission Confers Long-Term Protection against CVD

Achieving remission of prediabetes was associated with an approximately 50% lower long-term risk of death from cardiovascular disease (CVD) or hospitalization for heart failure, with benefits persisting for decades in two landmark diabetes prevention cohorts. In post hoc analyses of the US Diabetes Prevention Program Outcomes Study and the Chinese DaQing Diabetes Prevention Outcomes Study, individuals who restored normal glucose regulation after lifestyle intervention had substantially lower event rates over 20 to 30 years of follow-up compared with those who did not achieve remission, even after rigorous adjustment for baseline differences. These findings were consistent across both populations and confirmed in pooled meta-analyses, demonstrating a durable “legacy effect” that outlasted the active intervention period. Together, the results suggest that targeting prediabetes remission—alongside established weight loss—may represent a powerful and novel strategy for long-term CVD prevention.
