A cross-sectional study comprised 14,828 metabolically normal adults 30 to 59 years of age with no known cardiovascular disease in South Korea showed a higher prevalence of subclinical coronary atherosclerosis (coronary artery calcium scores ) in metabolically healthy obese (MHO) compared to metabolically healthy normal weight participants, supporting that MHO is not a harmless condition. The association was mediated by metabolic risk factors, such as LDL cholesterol, at levels below those considered abnormal. The results are consistent with a recent systematic review and meta-analysis, which concluded obese persons are at increased risk for adverse long-term outcomes even in the absence of metabolic abnormalities compared with their normal-weight counterparts.
